Handing in of cash and Registration of Accounts with Financial Institutes
This form has to be completed by all 'natural persons', who dispose over cash in Reichsmark or Rentenmark or in banknotes issued by the Allied Military Authorities, or those who have an account registered in their name with a financial institute. All sums of cash and bank accounts in Reichsmark owned by a family have to be listed on the same form, as family members are regarded husband and wife as well as children who, on the date of the currency reform, have not yet reached their 18th birthday. Only those who are not members of a family in the prescribed sense have to fill out a form of their own. It is irrelevant in which household they live.
In case, because of family members living at different locations, it is not possible to fill out one form per family, the members separated from their family are entitled and obliged to fill out forms of their own, with special consideration of paragraphs 5 and 6.
All questions are to be answered, with "yes", "no" or "does not apply". The form may be filled out and handed in by a representative, charged by written authorization. In this case the data have to be given not for the representative, but for the represented.

1. Information about the person, which hands in cash or which registers bank accounts, and on the members of his/her family. Family members for which information can not be given under numbers 2 and 3 because they are separated, have to be listed under number 6.

2. All Bank Accounts in Reichsmark, owned by the persons mentioned under number 1.

3. Total amount of the cash to be handed in by the persons listed under number 1.

4. Total amount of cash and of the combined deposited savings. In case the precise sum on the account, on the day when the currency reform takes place, is not known, give information as precise as possible.

5. Residence of the head of the family (only to be filled in, if he is unable to fill in the form at his residence; to be filled in by members of his family who have to fill in a separate form because they live separated from their family)

6. Names of family members who, because they live separated from their family, are not listed under number 1 (including a perhaps absent head of the family).

7. Revenue office where the last declaration of income or property has been made.

8. In case this form was handed in at a provisional exchange, and no account exists at a financial institute, here a financial institute has to be named where the cash amount is to be handed in.

9. In case the form is signed by a representative authorized in writing, here his name and address have to be given.

I assure to have given the preceding data according to my best knowledge. I am aware that intentionally made false information may be punished by a fine of up to 50,000 Mark and with a prison term of up to 5 years.

Herewith, the reception of the cash amount listed under number 3 and of copies number 1 and 2 of the form is confirmed.
